Registration

To receive a Registration Packet, click on Registration Request.

The Registration Form will be emailed back to you.

If you are not using Outlook for email then:

• Prepare an email using your system,
• The "to" address is "rollingf@pitel.net",
• Set the Subject to: "MNLA--Request Registration"
• In the Body enter: "your name, email address, mailing address, and (optionally) phone number"
• Also let us know if you can't open attachments, the Registration Packet can be mailed to you.
• We send the packet in both "pdf" and "doc" file formats.
• The Character Data Sheets (CDS) are also sent in both "pdf" and "doc" file formats. Complete using Word and return as an attachment to an email addressed as above.
• You can complete the CDS by hand; please include a check for your participation fees; and mail to:
